Appeal Form

The appeal form may be filled out if there is a discrepancy involving a Library fine, fee, or replacement cost. All patrons receive a date due notice at the time of check-out. Fines/fees are generally not waived due to:

• Lack of knowledge of library policy
• Non-agreement with library fine or fee structure
• Material loaned to another person
• Non-receipt or late receipt of library reminder notice
• Being out of town
• Forgetting the due date
• Not being able to get to library to return the item
• Financial hardship

If an item appears on your Library account that you believe you have already returned and you are being charged for it, please submit a Not on Shelf form and the Library will search for it.
